        The sample letter provided below is intended only to provide a suggested format for an evaluation request to your child's school district. A compelling evaluation request should contain detailed, specific information about your particular concerns and the difficulties your child is experiencing. Child advocates Pete and Pam Wright of Wrightslaw have authored a great series of articles on The Art of Writing Letters that promotes a non-confrontational "Story-Telling Approach" that is an excellent resource. Additionally, the Story-Telling Approach is also an empowering strategy that provides a record of the parents concerns, actions and communications that can be extremely useful and valauble in any proceedings that may arise between the parents and the school district.

Because my child has been experiencing some difficulties in school and I am requesting and hereby give consent for an evaluation to determine eligibility for services under the Individuals with Disabilities Education Act and/or Section 504 of the Rehabilitation Act of 1973.

I am particularly concerned about: <List all areas you see producing problems at school that you wish to be considered, such as failing grades, inattentiveness, distractibility, inability to follow directions, problems completing school work, disorganization, behavior or social problems, etc.>.

I understand that under state regulations, I am a member of the multidisciplinary team who will meet to discuss the results of the evaluation and be responsible for making a determination as to whether my child should receive Exceptional Student Education services. I will be happy to provide information about my child's history, strengths and needs to other members of the team.

If after consideration of my request you decide that an evaluation is not appropriate, please inform me, in writing, of the basis for your decision at your earliest convenience to the address provided above. Please also include information on procedural safeguards I may be entitled to pursue.
